About Primary Chest Wall Tumors

Primary chest wall tumors are a heterogeneous group of tumors arising from bone, soft tissue or cartilages of the chest wall. They can be classified based on their tissue of origin and their benign or malignant nature.

Approximately 50% to 80% of chest wall tumors are malignant. They may arise from bone or cartilage (approximately 55%) or soft tissues (approximately 45%).

Computed tomography of the chest provides excellent information on size, location, bony involvement and local infiltration of the tumor. A core needle biopsy or an incision biopsy for larger lesions (>5 cm) is the preferred method for preoperative histological diagnosis.

Surgery represents the cornerstone in the treatment of primary chest wall tumors.

Types of Chest Wall Tumors

Benign Tumors

Non-cancerous tumors with good prognosis when completely removed.

  • Osteochondromas
  • Chondromas
  • Fibrous dysplasia
  • Desmoid tumors

Malignant Tumors

Cancerous tumors requiring aggressive treatment and reconstruction.

  • Soft tissue sarcomas
  • Chondrosarcoma
  • Ewing's sarcoma group
  • Osteosarcoma

Surgical Treatment Principles

The basic principle of surgery for chest wall tumors consists of 3 components:

Radical Resection

Adequate 3-dimensional radical resection with clear margins to ensure complete tumor removal

Chest Wall Integrity

Restoration of chest wall integrity and stability to preserve lung function and protect vital structures

Cosmetic Reconstruction

Acceptable cosmetic reconstruction to restore appearance and patient confidence

Treatment Approaches

Surgical Resection

Complete tumor removal with appropriate margins.

  • Wide local excision
  • En-bloc resection
  • Clear margin assessment
  • Frozen section analysis

Reconstruction Techniques

Advanced methods to restore chest wall function.

  • Mesh reconstruction
  • Muscle flap coverage
  • Bone/cartilage grafts
  • Prosthetic materials

Diagnostic Workup

Comprehensive evaluation before treatment.

  • CT chest with contrast
  • Core needle biopsy
  • Incisional biopsy (>5cm)
  • Staging studies

Multidisciplinary Care

Coordinated treatment approach.

  • Surgical oncology
  • Medical oncology
  • Radiation therapy
  • Plastic surgery consultation
